# Assumption of Risk & Release of Liability
Ice hockey is a contact sport that carries inherent risks of injury. By signing below, the parent or legal guardian acknowledges and accepts those risks on behalf of the minor participant.
## 1. Acknowledgement of Risk
I understand that ice hockey involves risks including but not limited to falls, collisions, contact with pucks, sticks, or other players, and resulting injuries.
## 2. Release
I release Anytime Hockey, its instructors, coaches, employees, contractors, and venue partners from liability arising from participation, except in cases of gross negligence.
## 3. Medical Authorization
In the event of an emergency, I authorize Anytime Hockey staff to seek emergency medical treatment for the minor participant.
## 4. Photo & Media Release
I consent to the use of photos and video taken during sessions for marketing and promotional use unless I notify Anytime Hockey in writing otherwise.
## 5. Cancellation Policy
Refunds are available up to 24 hours before a scheduled session. Within 24 hours, credit toward a future session may be issued at Anytime Hockey's discretion.
By signing, I confirm I am the parent or legal guardian of the participant listed and I agree to these terms.
